An employee was loading a paving machine onto a trailer. The machine began to slide back off the trailer, and the employee jumped from the trailer to the ground to avoid it. He suffered a broken foot/heel, requiring hospitalization and surgery.
An employee was participating in a fire drill. During the drill, a firefighter asked the employee what she would do if she smelled smoke. As the employee was going to the fire pull station, she twisted her left leg and fell to the floor, resulting in a broken femur.
