An employee was clearing a jam in a gear machine. The employee cleared the jam using a small torque wrench, but another part jam occurred right behind the original jam. After unjamming those parts, the employee used a screwdriver to hold the parts back and his left index finger to prevent the next parts from coming down the track. One of the parts was released and caused the sensor to call for another part. The gate came down and cut his index finger resulting in a partial amputation.
An employee was walking behind the store with another associate to get paperwork from the delivery driver when she fell on the walkway and sustained a leg fracture that required surgery.
