Final narrative
An employee was servicing an automated guided cart, reported to have a hydraulic leak. The cart was on a lift about 6 feet off the ground. The employee reached under the raised platform and disconnected the hydraulic line, at which point the unit came down on their left hand and right index and middle fingers. They suffered a dislocation to the left wrist and lacerations to the right index and middle fingers. The employee was hospitalized and required surgery.

An employee was trying to remove a wheel from a rear wheel manipulator. The air cylinder holding pressure to the wheel became stuck. The employee worked to remove the wheel while the power was off; it was then turned on, and the employee's left little finger was amputated to the first knuckle. The machine was not locked/tagged out at the time of the incident.
