At about 2:15 p.m. on November 9, 2023, an employee was operating a hose from a concrete pump truck while working from the top of the form work for a foundation wall. The hose became clogged and bucked due to backpressure. The employee was thrown from the top of the form work and landed on the ground below, suffering a broken left femur. The employee was hospitalized.

On September 13, 2017, at approximately 2:30 p.m. an employee was spraying a form with oil. The employee slipped and fell about 8 feet and was caught under the arm by a form rack. The employee suffered a cut to the arm that severed a nerve.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
