On March 22, 2023, an employee was using a broom to push down on a dock plate. The employee lost her balance and fell from the dock to the ground, sustaining fractures to the left leg, left knee, and right heel.

On February 6, 2023, an employee slipped on a newly washed floor and fell, suffering a broken left kneecap. She was hospitalized.

An employee was cleaning a roller conveyor when the employee's finger contacted the chain, resulting in the amputation of the left small fingertip.

On November 22, 2019, an employee was walking to their next jobsite with a maintenance cart. A forklift struck the maintenance cart which then struck the employee. The employee fell to the concrete floor and sustained a broken right hip.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

An employee tripped over a bin and fell to the floor. She landed on her hip and braced herself with her arm, resulting in a fractured hip and wrist.

An employee was reaching for a piece of angle iron when it fell onto his hand, which was on a table to support him as he reached for the overhead rack. He suffered a broken finger and was hospitalized, requiring surgery.

An employee was putting dough into a forming machine when their left little fingertip was amputated by the forming die.

An employee was working to open the liftgate on the back of a delivery truck. His left middle finger was pinched between the gate and its frame, resulting in a fingertip amputation.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
