The employee's right foot was caught under the battery cover of the MHE. The employee sustained a chemical burn from the battery and a fractured right ankle.

An employee was driving an unloaded forklift outside a building. During a left turn, the forklift drove off the paved area onto an unpaved surface, became unstable, and tipped over. The employee was pinned under the forklift and suffered two broken leg bones and a broken hip.

The backhoe drove over a soft spot of dirt and began to tilt. The employee jumped off the backhoe, which continued to tilt and rolled over onto its side. The employee was caught between the rollover protection structure of the backhoe and the ground, resulting in a fractured pelvis that required surgery.

An employee was trimming tree limbs from a bucket truck. The employee fell from the bucket to the ground 16 feet below, suffering a concussion and a broken rib.
