An employee was on horseback roping a calf to be branded. The horse jumped and the employee fell to the ground. The employee was hospitalized for multiple broken ribs.

A crew was preparing a 20-foot section of 12-inch high-density polyethylene (HDPE) pipe to be swung into a welder using an excavator, where it would be welded onto a longer run of pipe. One employee was facing the longer pipe in the welder while the 20-foot section was being rigged. The 20-foot section moved and struck the employee's left knee. The employee's left leg was broken below the knee.

An employee was struck by a car backing up in the parking lot, resulting in broken bones in the leg and a severed artery. The leg was medically amputated above the knee.

An employee was realigning a "rover cart" on a track when they were struck from behind by a "rover cart" on a different track. The employee sustained a fractured right leg with a punctured vein.

An employee was on an extension ladder, using wire cutters to remove zip ties from a pivot to unroll the drop hose. The ladder shifted/slid on the pivot piping and the employee fell to the corn field ground/corrugate. The employee sustained lacerations to their liver and kidney. They also sustained fractures to both wrists and the left trochanter and required surgery.

An employee was cutting corner boards on a cutting board. As the employee brought the blade down, their left thumb tip was amputated. The machine was not guarded at the time.

A cable on a manlift came off the pulley. The injured employee grabbed the cable to place it back on the pulley. The manlift dropped, the cable was pulled tight, and the employee's left middle fingertip was crushed, resulting in an amputation.

An employee was attempting to stop tools from falling into product when their right hand was lacerated by the blades in the dough chunker machine.
