An employee was operating a standup forklift in a racking aisle when the vehicle collided with a guardrail. The employee's left foot was caught between the forklift and the guardrail, resulting in a left foot/ankle crush injury. The employee's left foot was surgically amputated and they were hospitalized.

An employee was standing at the back of a powered industrial truck (PIT), entering information into the vehicle-mounted computer. Another PIT passed by him carrying a double-stacked rack. The rack pinned his right leg against his own PIT. His leg was fractured and he was hospitalized, requiring surgery.

At about 2:30 a.m. on August 30, 2025, a DHL employee was operating a reach truck in a warehouse. The truck drove into the elevated forks of another, parked reach truck, and the employee suffered a broken left tibia. The employee was hospitalized, requiring surgery.

An employee was troubleshooting the superstack machine. Their right index finger became caught between the strap and anchor of the machine when the strap was tensioned, resulting in a fingertip amputation.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
