A butcher was cutting beef bones on a band saw when his hand slipped and the blade cut the ring and little fingers on the palm side of his left hand. Skin was removed midway down the fingers.

An employee was collecting carts in the parking lot using a cart pusher. The employee then used a remote control to move the line of carts through the front entrance door and into the vestibule. As the employee neared the rows of carts that were already in the vestibule, he attempted to move a misaligned cart out of the way of the carts he was bringing in and became pinned between the carts that were already in the vestibule and the carts he was bringing into the store. The employee sustained two broken knees.

An employee fell from a fixed ladder onto a concrete floor while retrieving product from a loft and suffered a fractured skull with neck and back injuries.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

An employee was working on a production line. The employee went to retrieve parts that came out of the parts holder, and their left hand became caught between a chain and a sprocket. Three fingers on the hand were amputated.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a pad printer. The employee's finger was caught in the printer and they suffered a fingertip amputation. The part of the printer that caught the employee's finger was unguarded at the time.

Employee 1 was investigating a gas odor. While employee 1 was checking the equipment, a gas explosion occurred, resulting in a fire that burned the employee's face. Employee 2 entered the area and a second explosion occurred, causing burns to their face and arms. Employee 2 was hospitalized.

An employee tripped while walking from one store register station to another. Her forehead struck a register, and she suffered fractures to two vertebrae in her neck. She was hospitalized.

An employee was pulling a display shipper using the provided strap. The strap broke and the employee fell to the floor, suffering a broken wrist.

The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.
