At approximately 10:15 a.m., an employee was unloading three rolling containers from a delivery truck. He was pushing one of the rolling containers onto the lift gate to stage it for lowering to ground level when it began to fall off the lift gate. The employee attempted to stop the container from falling, which resulted in an amputation to his right index fingertip.
