An employee was using a line level to keep tension in a wire line. The wire became stuck and needed to be uncoiled. While the employee was uncoiling the wire, the wire pushed his left hand against the level, crushing his left middle finger. The employee sustained an amputation to the left middle finger.

An employee was installing power lines from a bucket truck. The employee became overheated and was later hospitalized for dehydration.

An employee was picking up a glove from the ground. A truck outrigger came down on the employee's left foot as it was being lowered, crushing and fracturing the foot.

A wooden pole loosened as it was being pulled down and smashed an employee's left hand. One of the employee's fingertips was amputated, and another had the skin and flesh stripped to the bone.

An employee was canting a pole (moving a power pole from the ground to a standing position) when the rope and cant hook slipped because they were wet. The employee fell backwards and put his hands behind him to catch himself. He landed on his right hand and broke it near the wrist.

An employee was repairing the suspension of a semi-trailer. He was installing an air bag to the suspension when his left ring finger was crushed between the air bag and the suspension mounting beam. The employee was hospitalized.

An employee was dislodging a towbar and hitch between two golf carts. The upper part of the employee's left index finger was pinched in the towbar/hitch, causing an amputation at the last joint.

An employee was loading a motor onto a trailer when he struck his left elbow against the radiator. His elbow was lacerated and became infected, resulting in hospitalization.

An employee was helping a crew move a section of a grain conveyor that weighed 600-lbs into the proper configuration. The piece of equipment was sitting on a transport cart. While turning the cart to align it, the piece of equipment shifted and the employee's right middle finger was pinched in a small gap between the equipment and the cart. The employee pulled his gloved hand away and suffered a fingertip amputation.

On May 16, 2025, an employee was performing iron work. He was sliding a 3/4" x 2' x 4' bent plate across another plate to position it for installation. The bent plate crushed his left middle finger between the two steel plates. The distal bone was fractured and required a revision amputation.

An employee was troubleshooting the control setup for a variable air volume box above a ceiling. The employee's hand came into contact with an exposed, energized 277-volt wire that had loosened from its terminal on the box's assembly. He was shocked and fell off a ladder. His left hand passed across the ceiling grid and was lacerated. He also suffered an electrical burn to the right hand and was hospitalized.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was standing on an A-frame ladder approximately 6 feet above the floor while he attached a suspended light fixture to the ceiling. The employee lost balance, and fell to the tiled floor below, sustaining blunt force trauma to his head.

An employee was inspecting the overhead area of a single-person lift and repositioning the lift through a double doorway. As they were transitioning through the doorway, they were caught between the doorframe and the vertical mast of the lift. The employee was hospitalized with injuries to the nose, left maxillary sinus, left orbital bone, and left jaw, with fractures at the maxillary sinus area.

The injured employee was performing routine maintenance on a fan belt/pulley. The injured employee and another person were moving the pulley back and forth manually to find a bearing issue. The injured employee's left ring finger got caught between the belt and the pulley wheel, resulting in a soft tissue amputation.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was shot in the leg with a pistol during an altercation with two people who had entered the store without permission.
