Two employees were making alterations to a steel plate (80 by 100 inches, weighing about 1,000 pounds) that sat over the top of a circulating pump. They had lifted the plate with a magnet, slid it slightly off the edge of the pump's surface, and then used a torch to cut pieces of steel off. When the magnet was released, the plate came down on an employee's right foot and fractured it.

An employee was polishing a 3/4" x 13 1/4" steel shaft in a lathe using emery paper. The emery paper got caught around the workpiece and the employee's right ring finger was amputated.
An employee moved a forging from the layout table to the centering machine using an overhead crane. The employee removed the crane s hook from the forging and then moved the crane away from the area. As the crane was moving away, the hook got caught on the centering machine. The hook came under tension and then let loose, striking the employee on the right side of his head.
