An employee was sorting sucker rods. His right hand was caught in the area where the rod hits up against the frame and his index fingertip was amputated just below the nail.

An employee was troubleshooting a breaking rack machine when they noticed a pin on the machine was sticking out slightly from its location. The employee was pushing the pin back in and the press cycled, catching his right index finger in a pinch point and resulting in amputation below the second knuckle.
A crew was installing drilled micropiles alongside an outdoor covered deck foundation. The injured employee was working the front of the drill when a loose section of casing dropped onto the tip of a rig wrench and pulled the wrench down to an embedded casing. The employee's left little finger was caught between the rig wrench and the embedded casing and was amputated above the top knuckle.

The load was lifted at an angle, causing the cylinder head to get stuck on the cylinder head studs. When it became free from the studs, the cylinder head swung. It struck the technician and caught them against the cabinet door. This resulted in an amputation of their left index fingertip at the distal knuckle.
