Two employees were lifting and stacking steel panels (for concrete forms) onto a trailer. The panels measured 2 feet by 8 feet. While stacking one panel on top of another, the injured employee's left index finger got caught between the two panels. The employee suffered crush injuries that required an amputation of the finger above the knuckle.
