An employee was rearranging unit stacks inside a trailer. He stepped onto one unit (5.5 feet tall) to push the unit behind it. The employee fell between the two units onto the floor of the trailer, resulting in five broken ribs on the left side and a punctured left lung.

An employee was removing a 6' x 6' cover from a log chipper using a hoist. The chipper disc contacted the cover which then dislodged from the hoist and struck the employee, resulting in lacerations to the head, neck and face and broken teeth.

An employee was repairing a sight glass on a water holding tank and noticed a leaking pipe that needed repair. He stepped onto a 3-inch pipe and his wrench fell, causing him to lose balance. The employee fell onto a mixer motor and landed on the floor. The employee sustained a fractured left wrist, fractured left clavicle, and five fractured ribs.

An employee was helping to remove a tote filled with ink when the tote contacted his left hand and pinched his finger between it and a metal frame, amputating his fingertip.

An employee was fixing a spacer on a stack of 2 x 10's when a few of the 2 x 10's fell on his left leg, breaking his tibia and fibula. He was hospitalized and had surgery.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.

An employee tripped over a concrete curb stop in a parking lot. The employee fell to the ground, landing on their left hip and breaking it.

On October 22, 2025, an employee was working to put a conveyor chain back on the track of a conveyor when their fingers were caught between the chain and the chain track, pulling their right hand into the sprocket. Four fingers were amputated.

An employee was cleaning the headers of the core machine using a hand drill with a brush attachment. His glove was caught on the brush shaft, resulting in amputation of the left middle fingertip.

An employee was operating a winder paper mill machine when their right index finger became caught in the machine. The finger was partially amputated at the first knuckle.

An employee was lubricating a chain and sprocket on a corrugator splicer. He lost his balance, and his right middle finger came into contact with the chain and sprocket. The fingertip was amputated at the nail bed.

An employee was entering a building and reached out to catch a closing door. The door closed on the employee's finger, resulting in the partial amputation of the fingertip and nailbed.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
