An employee was on an A-frame ladder fixing an electrical wire on an HVAC unit in a ceiling. The employee was holding onto the unit with their left hand when the employee was shocked with 480V of electricity. The employee let go of the unit and fell 6 feet from the ladder. In the midst of falling, the employee hit their head on a metal work station. The employee was hospitalized for two skull fractures, burns to three fingers on their left hand and one finger on their right hand, and unspecified right arm pain.

On 10/12/18, an employee was moving a large loom that was mounted to a table on iron caster wheels when one of the caster wheels broke, causing the loom to tip forward and strike the employee's chest. The employee was hospitalized.

An employee was performing maintenance on a conveyor belt when his right forearm became caught and fractured in the conveyor.

The employee was hospitalized with six fractures, including her forearm, index finger, ring finger, wrist, and thumb, as well as a severed artery. The machine was not guarded or locked out/tagged out at the time.

An employee was repositioning merchandise after the floor was stripped and waxed when they slipped on uncured wax and fell to the floor. The employee sustained a fractured right femur.
