On March 21, 2024, an employee was descending the indoor, carpeted stairs of a patient's residence when their heel slipped on the second-to-last step and they fell down the steps onto their left side. This resulted in injuries to their right knee, left shoulder, and bruising on their left big toe.

On January 15, 2019, at 8:45 a.m., an employee was walking into work from where she parked on the street when she slipped and fell, striking her head on the icy sidewalk. She was hospitalized with a fractured occipital bone.
