An employee was cleaning a steam kettle; he had the floor grate lifted to access the drain area. He tripped on the open floor grate, striking his knee against the steam kettle's drain pipe and falling onto multiple pieces of kitchen equipment. He landed on the floor, where he struck his head. He was hospitalized with lacerations to his ear, which required surgery to repair.

An employee was removing the access panel to drain cold water from the hot well. As the employee bent over to remove the panel by the two access slots, she became light headed. Upon standing up, one her fingers remained in the access panel and the panel fell to the ground, pulling the employee to the ground with it. The employee sustained a hip fracture.

An employee was cleaning out the dishwasher when he suddenly fell to the ground and hit his head, becoming unconscious due to dehydration.

An employee was walking up to a truck and walked into the open lid of a toolbox. The employee sustained a fracture to the corner of their right eye.

The employee heard a rumbling sound coming from a low-pressure steam vent and when he looked up, he saw water falling from the vent. He quickly moved to avoid the falling water and ran into a pipe and then fell, resulting in a right tibia fracture.

An employee had been operating a forklift equipped with a clamp attachment. He was transitioning to another powered industrial truck (PIT) when he struck his left knee on a part of the equipment and sustained a laceration to the knee requiring sutures and hospitalization.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.

On September 29, 2025, an employee was removing a pan of pasta from the stove when the pan tipped over. Hot water contacted her legs and feet, resulting in burns to both feet that required hospitalization.

An employee was walking through a passageway sideways when the edge of her shoe got caught and she fell onto her right hip, resulting in a broken femoral head.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
