An employee was walking in the break room, tripped on a floor mat, and fell to the floor. The employee sustained a fractured right kneecap and blood clots.
An employee was moving the porcelain portion of a capacitive voltage transformer into a steel bin using a forklift with lifting straps. The transformer slipped off the forklift and crushed her right hand, which required surgery. She also sustained a right shoulder fracture.
