An employee slipped on water in a hallway and fell to the floor, landing on his left side and head. He sustained a back strain/sprain and was hospitalized.

An employee was walking near a checkout aisle when she tripped over a box of bags on the floor and fell to the floor. The employee was hospitalized with a fractured right femur and required surgery.

An employee was on a 6-foot ladder working to adjust a camera system around the ceiling tile. The ladder buckled and the employee fell to the floor, sustaining a laceration to his head and a compression fracture to his lower back. The employee was hospitalized and required staples for the laceration.

An employee was working at a cash register. She turned away from the register, took a step or two, tripped, and fell to the concrete floor. She suffered a broken left knee and was hospitalized, requiring surgery.

An employee was pushing a pallet with a motorized pallet jack when she stepped on a flattened box on the floor, slipped, and fell, fracturing her right hip. She was hospitalized and required surgery.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

An employee fell from a loading dock to the ground 2-3 feet below. She suffered a compound fracture of the left hip and was hospitalized, requiring surgery.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was walking across a sales floor. The employee and a customer collided at a corner; the employee fell and broke their right kneecap on the floor. The employee was hospitalized, requiring surgery.

An employee was operating a powered industrial truck (PIT) when it struck a steel rack and he fell from the PIT to the concrete floor. The employee was hospitalized with a fractured left leg and required surgery.

An employee was standing on a ladder, using a stick to push a blanket to unblock a tunnel. The ladder started to shake and the employee fell from the ladder, landing on his back on the concrete floor. He sustained a fracture to his lower back, pelvis fractures, and a concussion.

An employee was unloading merchandise from a trailer using a pallet jack when he became caught between the pallet and the rolling tracks of the pallet jack. The employee was hospitalized with a fractured bone in his leg and required surgery.

On May 3, 2025, an employee was getting an item from the freezer when she tripped on a pallet that was behind her and fell, striking the ground. The employee was hospitalized with an injury to the lower right side of her body and muscle spasms.

An employee fell while moving a water canister, resulting in fractures to four fingers on her right hand.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
