An employee was descending a dirt road on a utility task vehicle (UTV) after finishing fueling a booster pump. The vehicle left the dirt road and went over a fallen tree. The employee sustained broken ankles, a sprained left wrist, and a contusion/scrape on his upper abdomen. The employee was hospitalized.

A driver pulled into the facility yard and backed their truck up to a dock. He was unloading an 85-foot articulating boom lift when the boom lift fell. He was slung out of the bucket of the boom lift and his self-retracting lifeline (SRL) broke. He impacted the outside of the bucket. The employee sustained a fracture to his heel plate and toes on his right foot.

On November 21, 2024, an employee was operating a truck-mounted forklift, with stabilizers deployed, to move a stack of dimensional lumber onto a truck. With the load of lumber raised, the stabilizers retracted and the forklift lost ground contact. This caused the forklift to abruptly lunge forward and the employee struck the interior of the roll cage. The employee was hospitalized with multiple head lacerations that required eight stitches in the front of his head and two staples in the back of his head, facial fractures, three cracked vertebrae in his neck and upper back, and a shin laceration.

An employee was operating a forklift to move a boat molding when the forklift turned sideways and pinned their left ankle. The employee sustained a fracture to their left ankle.

An employee was painting while in an aerial boom lift. The employee sustained facial fractures while operating the lift over rough terrain.

Two employees were working on a forklift with its mast in the air when a chain came loose and the carriage fell down on them. The injured employee was hospitalized with a fractured left ankle, a laceration on the left side of their forehead, and a fractured L1 vertebra.

An employee was operating a machine used to roll erosion control fabric. Her left hand was caught and crushed in the rollers. She was hospitalized.

An employee was using a pipe grooving machine to groove an 8-inch pipe. His gloved index finger on his left hand was caught between the groover and the pipe, crushing the top part of his finger. The employee's left index finger was partially amputated.

An employee was cutting electrical wires when an arc flash occurred, resulting in third-degree burns to their hand.

An employee was being trained on operating a powered industrial truck (PIT) with a side-saddle stance. The employee was instructed to move the PIT s joystick back and forward to get a feel for it. The PIT went backward through a dock door that was approximately 3 feet off the ground. The PIT ended up on the employee. The employee was hospitalized with an amputation below the left knee, third-degree burns to lower extremities, and multiple fractures to the lower extremities.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
