An employee was unloading diesel fuel from one compartment in a tanker to the next compartment. Fuel was released when a valve was opened. When the employee tried to stop the flow, some of the fuel entered his mouth. He became ill due to fuel ingestion and was hospitalized.

An employee was climbing into the cab of a semi-tractor when he stepped on the battery cover and it shifted from under him, causing him to fall to the ground. The employee sustained a fractured left wrist.

A panel was being lifted when an attached roller (approximately 20 pounds) slipped and fell about 2 feet onto the hard hat of an employee working below. The roller also struck the employee's shoulder and back. The employee was hospitalized and suffered a contusion to the shoulder/back.

Employees finished loading a tanker trailer. The injured employee entered the cage area on top of the trailer to tighten the dome lid when the truck pulled out of the loading area. This caused the injured employee to fall from the trailer to the ground below, resulting in a fractured leg.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
