Final narrative
A panel was being lifted when an attached roller (approximately 20 pounds) slipped and fell about 2 feet onto the hard hat of an employee working below. The roller also struck the employee's shoulder and back. The employee was hospitalized and suffered a contusion to the shoulder/back.

An employee was sanding a piece of metal on a belt sander when his gloved hand was caught between the belt and the piece being sanded. The employee sustained a partial amputation to their right index finger.
At 7:30 p.m. on January 28, 2021, an employee was sitting in his vehicle in a company parking lot when he was shot in the face with a shotgun through the driver's side window. He was hospitalized, undergoing surgery.
Employees were in the process of draining material from a mixer through its pneumatically actuated ball valve, which is approximately 3 inches in size and located at the bottom of the mixer. The ball valve was actuated, but material did not flow. The injured employee was trying to un-clog the ball valve when it actuated and amputated the employee's left middle fingertip.
