An employee was walking to a break area when she was struck and run over by an industrial forklift. The employee was hospitalized with an injury to her body and a severe leg injury.

A drive chain broke at the wave feeder on the log line infeed. The injured employee came to help move logs with a remote-controlled hoist so they could reach the chain sprocket. He connected the log grapple to a log and started to lift the log with the hoist. The log rolled and pinned his left leg against another log, fracturing his leg. The employee was hospitalized.

He was holding onto the hoist cable for support as he entered the sorter bay when the lumber racks in the bay slowly went down, causing his left hand and fingers to become caught between the hoist cable and pulley. The employee sustained fractures to his middle, ring, and little fingers and an amputation to the ring fingertip.
