An employee was cleaning a V roller on a line when the employee's right middle finger was caught between the rollers, resulting in a fingertip amputation. The power was on at the time of the incident.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.
