An employee was managing a prescribed burn when the fire behavior increased and he retreated to the road. His tool and foot got entangled in vines which slowed his retreat, and the fire encased his route back to the road. This resulted in blistering and second-degree burns to both elbows, both knees, the back of one thigh, the inner thigh, and his nose.

An employee's right middle finger was crushed by an excavator bucket, resulting in an amputation.

An employee was unloading a trailer when their left thumb was caught between parts of the trailer body, resulting in a partial amputation.

An employee had been felling hazardous trees in a campground. The top of a tree detached, fell, and struck the employee. The employee sustained injury to the head, shoulder, and arm.

While actively suppressing a spot fire, a forest service employee was moving downhill when they lost their footing and fell contacting a large rock which resulted in a fractured femur.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

An employee was attempting to lift a steel beam from a horizontal, stacked position to a vertical position using a crane. They had placed two hooks on the east side of the beam on the flange. As the employee was hoisting the crane, the beam began to shift and pulled the employee forward onto the stack. When the material shifted, the hooks released and the material fell, pinning the employee between the two beams. The employee was hospitalized with soft tissue contusions on their proximal right thigh and interior left thigh.

An employee was moving a tensile strength test device with a dolly. The device fell from the dolly onto the employee's right leg, causing a compound fracture.

An employee was rigging I-beams. An I-beam slid off the stack of I-beams and fractured the employee's leg.

An employee was using a chainsaw to cut downed trees. The employee slipped and fell, and the chainsaw lacerated their left hand.

A crew of approximately 20 employees were constructing hand lines when a dead tree collapsed and injured three of the employees, including two hospitalizations. One employee was hospitalized due to shoulder injuries, and the other employee was hospitalized due to collapsed lungs, spinal injuries, and a broken tibia and fibula.

An employee was cutting brush and shrubs. A bee stung the employee on the base of their neck. The employee sustained an allergic reaction and was hospitalized.

An employee was engaged in initial smokejumper training which includes physical fitness testing, tree climbing training, chainsaw and crosscut certification, as well as initial introduction to equipment and smokejumper-specific safety procedures. Some of the training involves carrying 110 pounds of equipment. The employee reported tightness in his chest and difficulty breathing. The employee was hospitalized with a lung injury.

An employee participated in smokejumper training, which includes carrying weight. He suffered from dehydration and was hospitalized with rhabdomyolysis.

An employee was working to remove a stuck strap from a tray line while on an A-frame ladder. They fell 6 to 8 feet to the concrete floor, resulting in a head contusion and fractured ribs that required hospitalization.

An employee sat on an unleveled bench outside the cafeteria and fell to the concrete. The employee was hospitalized with a head contusion with bleeding and injuries to the right side of her body, her right hip, and right hand, as well as a scrape to her elbow.

A driver was exiting his truck and fell from the bottom step to the ground. He fell on the pavement on his left side, breaking his hip.

An employee was walking to his car when he tripped over a board that was secured to the floor of the dock area. His left knee struck the cement dock. The employee was hospitalized and required surgery to repair a broken knee.
