An employee had been using a forklift to offload steel crates from a truck. The employee was closing the panel door of a crate that was unloaded and had been set aside as another forklift was pushing the crates when the employee's right index finger became caught between the panel doors of the crates. The finger was crushed and lacerated, requiring surgery.
