The injured employee was holding a piece of glass and adjusting it in place when another piece of glass was pushed toward the injured employee. His left index finger was caught between the two pieces of glass, removing the fingertip.

On March 28, 2023, an employee was changing ceiling tiles while working from a 6-foot A-frame ladder. The employee felt an electric shock, causing him to fall approximately 4 feet to the ground. The employee sustained a lower back injury.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was diagnosing the lack of flow of product to a powder bin. The employee removed the rotary star valve below the bin. While he was reinstalling the valve, his right middle finger was crushed between its shaft and its housing. The finger was partially amputated.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was conducting routine maintenance on a mixer. New lid pins had been installed, and the employee was rotating by hand to ensure they had been installed correctly. The employee's left middle finger was caught between the lid and top of a pin. The momentum from the mixer continued, causing a crushing injury to the finger. The employee underwent a medical amputation from the top knuckle to the tip of the finger.

An employee was working on a platform, raised 6 to 8 feet in the air, to get something off a rack. The employee fell from the platform to the ground and sustained a head injury and injury to multiple body parts.

An employee was rolling two stacked trunks (on wheels) toward a forklift during load-in for a concert. The trunks toppled over and struck the employee, resulting in hospitalization with fractures to their nose and femur.

An employee was moving a ticketing scanner cart back to the box office when they tripped and fell forward, striking the left side of their head on the base of a refrigerator. The employee had pain in their left knee, shin, arm, and shoulder. The employee was hospitalized with a concussion.

An employee was working on mechanical repairs to a tractor that was on a 22-ton hydraulic lift/jack. The employee was loosening bolts on the rear differential when the jack failed/slipped, and the tractor came down, pushing the employee's drill to the ground. The employee's left hand was caught under the drill with the weight of the tractor pushing down on it. Two of his fingers were lacerated and he was hospitalized.

An employee was using an electric pallet jack to unload a pallet from a delivery truck. While he was turning the pallet jack to move it backward, it sped up, spun, and collided with a loaded, stationary pallet on the ground. His right foot was pinned between the loaded stationary pallet and the pallet jack. The foot was lacerated and he was hospitalized.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.
