The injured employee was collecting waste and depositing it into the front bin of the garbage truck while on the collection route. The truck ran over and crushed the employee's right ankle. The employee sustained an open lateral dislocation of the right subtalar joint and a Lisfranc injury and was hospitalized.

An employee was stopping traffic at a job site so a piece of equipment could be moved to another location when he was struck by a car. It sent him in the air and he landed on his back, resulting in hospitalization with five fractured ribs, a fractured forearm, and a liver injury.

Trego Excavating Inc.

An employee was repairing a tri-axle dump truck on the side of the road when the clutch was released, causing the truck to lurch forward over the wheel chocks. The employee was crushed underneath the first set of rear wheels and sustained a fractured lower left leg and a knee injury.

An employee was performing road assistance activities and was in the process of towing a vehicle when they were struck by another vehicle. The employee sustained injuries to their back, right clavicle, arm, pelvis, hip, and leg, resulting in hospitalization.

An employee was collecting waste in a residential area on his route and was struck by a motor vehicle while walking toward the driver's side door from the rear of the truck. The employee was hospitalized for a left fibula fracture, a left lateral malleolus fracture, and a hematoma above his right eye.

James J. Bourke Services, LLC

Two employees were walking on either side of a street, making sure that lengths of 6-inch-wide sewer pipe did not slide off the forks of a skid steer that was carrying them down the road. The skid steer's rubber tracks ran over one of the two employees' feet. He suffered foot and ankle fractures.

Rumpke - Beech Hollow Landfill

A driver had backed up a rear load truck to a dumpster. The winch cable was bound, and another employee went to use the controls to put slack in the line. The cable tightened instead of loosening while the driver was holding it, and it caught his hand against the truck and crushed his thumb. The thumb was fractured and lacerated; he was hospitalized, requiring surgery. The incident took place during a training activity.

An employee left the guard shack to speak to a customer. A bulldozer ran over her right foot, resulting in three broken toes.

An employee was pulling into a customer's parking lot to pick up a roll-off container when the vehicle ran over the median curb. The vehicle then veered to the left, striking the side curb and the retaining wall. The employee sustained injuries to his neck and spine, as well as a damaged artery, a head laceration, and fractured ribs.

An employee was replacing the bracing arm of a baler. The replacement part (weighing an estimated 500 pounds) was moved into place using a truck-mounted crane with a lifting strap attached to the designated pick point. The employee then used his right hand to stabilize/align the part. The part shifted, crushing his right index finger against the mounting pin hole. The employee's finger was amputated above the first knuckle.

An employee was moving a mower with a tractor bucket when the bucket slipped and struck another employee on the hand. The injured employee sustained a partial amputation of their left little finger.

An employee fell while moving a water canister, resulting in fractures to four fingers on her right hand.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
