Pennex Aluminum Company LLC

An employee was removing a transfer bolt from a 14-inch casting table using his left hand. As he unscrewed the bolt, the platen under the table was lowered. It pulled the screw down through the table top port, smashing the employee's left index and middle fingers between the bolt washer and the table. The index fingertip was amputated and the middle finger was crushed.

PENNEX ALUMINUM COMPANY, LLC

An employee was operating the manual side of a press stretcher (the tail side). He placed a block inside the channel of the aluminum extrusion and clamped it in the jaw end of the stretcher. When the employee noticed the block was out of alignment, he opened the jaw with his right hand and used his left hand to re-align the block. While he was re-aligning the block, the jaw handle slipped from his hand and closed on his finger. This occurred at the same time the head stretcher began pulling on the extrusion. The employee's left index finger was caught between the jaw and the extrusion. He pulled his hand back and lost his fingertip.

An employee was manually adjusting a finishing saw stop when her right thumb was caught between the stop and the roller rail. She suffered an amputation to her right thumb (nail area).

Pennex Aluminum Company LLC

An employee was operating a forklift, carrying an aluminum billet. Another employee was acting as a spotter for the forklift driver. The forklift operator placed the aluminum billet onto the storage rack and began moving away from the area. As the forklift operator was moving back from the area, the forks of the forklift caught another aluminum billet. The aluminum billet rolled off the storage rack and struck the employee who was acting as a spotter. The employee suffered a broken tibia and fibula. The employee was hospitalized for surgery on the broken leg.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

Thompson Hardwoods, Inc.

An employee and co-worker were working to unjam and move a 100-pound piece of crossed-up cant timber. The employee lost their grip and the timber fell on their right hand, resulting in crush injuries to the fingers.

An employee was working on a cushion ring (steel). As they lifted the cushion ring with both hands to flip it up, the ring slid down and the employee's right middle fingertip was crushed between the ring and the work table. The employee was hospitalized with a fracture, and their right middle fingertip was amputated.

HEXPOL Compounding PC, Inc.

Two employees were lifting a roll of steel poly bar (50 inches and approximately 150 pounds) from ground level to a height of 4 feet at the 66-inch rubber calendar. One employee dropped the roll, resulting in the poly bar contacting the injured employee's right knee. The employee was hospitalized with fractured right patella.

Macktown Construction Group

The injured employee was walking in front of the property by the front door as another employee was removing debris from the roof. The injured employee was struck by a 2 x 4 that was being cleared from the roof. He was knocked unconscious and sustained brain bleeding, as well as head and facial injuries, and required stitches.

An employee was adjusting a stretcher. Their foot went between a 3-inch opening on a press. The employee sustained an open fracture of their lower left leg above the ankle.

An employee was standing at a press machine, removing metal from a die cradle that was stuck. Once the metal was removed, the side die closed on his right wrist and lower forearm, breaking bones in the arm.

Elixir Extrusions, LLC

An employee was pulling metal using a tail stretcher. He pressed the double palm buttons to start the process when the clamp became stuck. The employee pulled on the metal. His left thumb was inside the clamp when it opened, cutting and amputating the tip of his thumb.

An employee was operating a 2 1/2 inch extruder press when aluminum became jammed in the press. Pressure built up and the piece of aluminum shot into the employee's left index finger, resulting in hospitalization and surgery.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
