While clearing a clog inside a pillow fill machine, a temporary employee's fingertip was amputated by the blade mechanism.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

An employee was working on a production line. The employee went to retrieve parts that came out of the parts holder, and their left hand became caught between a chain and a sprocket. Three fingers on the hand were amputated.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a pad printer. The employee's finger was caught in the printer and they suffered a fingertip amputation. The part of the printer that caught the employee's finger was unguarded at the time.

An employee was struck by a forklift and knocked to the ground. The employee was hospitalized with hip and sacral fractures.

An employee was moving a large metal toolbox when it tipped over, struck the employee, and pinned him to the ground. He suffered a broken hip and rib.

An employee was working on a potato piler. The employee's left hand came into contact with a live electrical line, and the employee suffered a third-degree burn.

An employee was monitoring the milling process when the flexible sock fell off a sifting machine. The employee put it back on and when he pulled his hand out, his right index finger was cut by a metal edge on the machine. The employee's right index finger was amputated. The machine was running at the time.

Two employees were cleaning a glass vessel when it ruptured. Employee 1 was hospitalized for abrasions/lacerations to the face, neck, and eyes as well as exposure to sodium amide to their face and eyes. Employee 2 sustained abrasions and lacerations to the neck and was not hospitalized.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
