An employee was horizontally pulling air pre-heater baskets with a come-along connected to a pad eye on the basket. The pad eye broke off the basket and flew toward the employee, hitting him on the forehead. He sustained a laceration, fractured skull, and hemorrhage.
An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.
