An employee was walking toward a stacker machine to get a strap off the rear tire when he was struck and run over by a stacker machine. He sustained multiple severe injuries including crushed legs, an injury to his lower left torso, and a lacerated left arm. The injuries resulted in an amputation to the employee's left arm and leg.
