Employees were using a powered industrial truck (PIT) to move steel dock plates into a scrap dumpster. The injured employee was giving hand signals/directions to the PIT operator when his hand became caught between the forks of the PIT and a steel plate. A finger was partially amputated.

A clamp truck driver trainee was setting down a roll of paper. The clamp struck the floor and the roll popped out of the clamps. It rolled forward and struck the employee, pinning him against another roll of paper. The employee suffered crushing injuries to the upper legs and was hospitalized.

As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was resetting a warehouse racking system utilizing an order picker. They fell approximately 5 feet from the order picker to the concrete floor and sustained fractured ribs.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.
