On January 7, 2025, at approximately 11:18 AM, an employee was operating a retainer carrier (RTC) to pick merchandise. He exited the RTC and was then struck by the RTC. His right leg was pinned between the rack and the RTC. The employee was hospitalized with a laceration to the right calf.

An employee was returning merchandise to a shelf in an aisle. The employee tripped over a stool and fell to the ground, suffering an injury to the right hip.

An employee was driving a double pallet jack down an aisle when another piece of equipment came through the aisle. Realizing that the two pieces of equipment were about to collide, the employee jumped off the pallet jack. It then ran over his leg, breaking his leg/ankle.

On November 18, 2023, at 6:07 p.m., an employee was walking out to the parking lot when he was struck by a truck and knocked to the ground. The employee suffered a broken leg and ankle that required hospitalization and surgery.

An employee was moving merchandise in boxes, rolling containers, and totes, from a truck to store shelves when they sustained back and neck injuries.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

An employee was disassembling a pressurized unit using a driver drill. When the unit was disassembled, it caused a kickback from the tube head, causing the drill to strike the employee's left forearm. They sustained a diaphyseal fracture to their left radius and ulna, which required surgery.

An employee was using a box cutter when it cut the tip of his left thumb resulting in a fingertip amputation.

While working at a customer's residence, an employee was cutting tape with a box cutter to wrap insulation for a piece of pipe. The box cutter struck a hardened object within the wall causing it to bounce back and puncture the employee's right eye. The employee was hospitalized.

The employee's socket slipped off the bolt and his left hand was crushed against the truss, resulting in injuries to the index and little fingers, requiring hospitalization, surgery, and amputation of the index finger.
