An employee was walking on a ceiling grid and fell approximately 60 feet through a draft opening striking another employee on a lower level. The employee on the lower level was hospitalized for fractures and other injuries. The employee that fell suffered injuries that resulted in a fatality.

An employee removed a high voltage cabinet and went to push a cable through when an arc flash occurred. The employee sustained burns on the right hand, left arm, and the back.

An employee was struck by a UTV while walking along a road on site, dislocating the employee's left ankle and fracturing the left fibula.

An employee was carrying a tent with three other employees when he stepped in a grassy area and fell, fracturing his right femur.

While descending a portable step ladder to reposition it, an employee missed the bottom rung and fell approximately 18 to 24 inches to the concrete floor below. The employee was hospitalized with a fractured right forearm, elbow, and pelvis.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

An employee fell from a loading dock to the ground 2-3 feet below. She suffered a compound fracture of the left hip and was hospitalized, requiring surgery.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was troubleshooting the control setup for a variable air volume box above a ceiling. The employee's hand came into contact with an exposed, energized 277-volt wire that had loosened from its terminal on the box's assembly. He was shocked and fell off a ladder. His left hand passed across the ceiling grid and was lacerated. He also suffered an electrical burn to the right hand and was hospitalized.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was standing on an A-frame ladder approximately 6 feet above the floor while he attached a suspended light fixture to the ceiling. The employee lost balance, and fell to the tiled floor below, sustaining blunt force trauma to his head.

An employee was inspecting the overhead area of a single-person lift and repositioning the lift through a double doorway. As they were transitioning through the doorway, they were caught between the doorframe and the vertical mast of the lift. The employee was hospitalized with injuries to the nose, left maxillary sinus, left orbital bone, and left jaw, with fractures at the maxillary sinus area.

The injured employee was performing routine maintenance on a fan belt/pulley. The injured employee and another person were moving the pulley back and forth manually to find a bearing issue. The injured employee's left ring finger got caught between the belt and the pulley wheel, resulting in a soft tissue amputation.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.

An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.
