An employee was dismantling a scaffold when he slipped and fell off the scaffold. The scaffold he was tied to then also fell. The employee landed on the ground 25-35 feet below and sustained multiple fractures.
An employee had just dismantled a hoist and was putting it in an upright position to remove the motor. As the track was lifted, the feet slipped out of the track, causing the motor to fall onto the employee's right leg and pin it to the ground. The employee sustained a right tibia fracture.
