An employee was sanitizing a conveyor belt with a wipe when their right hand fingers were caught between the gauging roller and the belt, resulting in partial amputations to three fingers.

An employee was on a three-step ladder fixing a motor on the incline conveyor. As they were stepping down from the ladder, the employee slipped and their foot became caught between the bottom and second steps. They were hospitalized for a shin fracture in two places.

An employee was unjamming two small boxes on the case maker when their finger came in contact with the sprocket of the conveyor. The employee's right ring fingertip was amputated. A guard was in place at the time.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

An employee fell from a loading dock to the ground 2-3 feet below. She suffered a compound fracture of the left hip and was hospitalized, requiring surgery.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee had been cleaning red garbage bins (gondolas) near the dock area. The employee tripped and fell off the dock. The employee sustained fractures to an elbow and two ribs, and had bleeding in their lungs.

On June 2, 2025, at approximately 8:45 PM, an employee was taking apart the trayoff belt on a conveyor. While working to remove the roller, the employee's left ring finger was crushed between the roller and the frame of the equipment. The employee sustained a partial fingertip amputation.

An employee tripped and fell over a conduit at a chocolate line and was hospitalized with fractures to both wrists and elbows.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
