An employee was pulling product from lower shelves in a warehouse when a gutter coil located on higher shelving became dislodged, falling on the employee. The employee suffered a lacerated right forearm, a broken pelvis, broken fingers on right hand, a scrape on their head, and possible broken vertebrae in their back.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was inspecting the overhead area of a single-person lift and repositioning the lift through a double doorway. As they were transitioning through the doorway, they were caught between the doorframe and the vertical mast of the lift. The employee was hospitalized with injuries to the nose, left maxillary sinus, left orbital bone, and left jaw, with fractures at the maxillary sinus area.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was operating a stand-up forklift. The forklift drove into a rack in a warehouse, and the employee's right hand was caught between the rack and an upright supporting the forklift's overhead guard. The employee suffered a fracture and laceration to the hand, just below the little finger.

An employee was helping to install a tarp on a roof when they fell through a skylight to the ground below, resulting in fractures to their face and ribs.

On August 5, 2025, an employee was exiting a room when his left middle and ring fingers became caught in the door as it shut. The middle fingertip was amputated.

An employee was driving a forklift and unloading racks of glass from a truck. A rack began to tilt; the employee exited the forklift and went to stabilize it. The rack tipped and pinned his left arm against the forklift. The forearm was broken and required surgery.

An employee was sitting on a stack of steel beams. When they stood up, the top beam fell onto the employee, resulting in a broken left leg.

An employee was working to tow a forklift with another forklift when the tow line came loose and struck their face. The employee sustained head, face, and eye injuries.

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.
