Final narrative
An employee was helping customers load 36x72 tile from a cart into their truck. The cart became unbalanced and fell. The tile fell off the cart and the employee tried to catch it. His right hand was pinched between the tile and the pavement, resulting in the amputation of his middle fingertip.

A forklift was backing out of an aisle when it pinned an employee against a support pole. The employee suffered a broken pelvis and internal organ damage.
The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over.
