An employee was walking between a closed dock door and a staged pallet of product. The floor was wet with condensation from the freezer area and the employee's foot got caught at the bottom of the pallet. While trying to free his foot he fell to the ground, resulting in a fractured right tibia.

He was about 16 feet up the ladder, which was not fully extended. The base of the ladder slid away from the house. The employee fell onto the porch roof, then to the ground below, a distance of about 10 feet. He suffered broken ribs, lacerations and facial abrasions, and a potential facial fracture.
