Final narrative
An employee acting in the capacity of a pile driver was greasing a pile driving hammer that was on the ground while in a blind spot of the crane operator. The hammer was raised and pinched the injured pile driver's left hand between the sidewall of the hammer and the grease gun in the piling leads, resulting in amputation of the middle fingertip through the nailbed and a fractured ring finger.

An employee was handling a cable that runs between a drum and the spud. He was trying to put slack in the cable line when the spud shifted and caused the cable to tighten. The cable struck and fractured his left leg just above the ankle.
