An employee was operating an unguarded hydraulic press. When the employee placed his hand inside the press to remove one of the molds, the employee's fingers on his right hand were caught between the mold and the top plate of the press. The employee was hospitalized, and four fingers were surgically amputated.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was working on a production line. The employee went to retrieve parts that came out of the parts holder, and their left hand became caught between a chain and a sprocket. Three fingers on the hand were amputated.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a pad printer. The employee's finger was caught in the printer and they suffered a fingertip amputation. The part of the printer that caught the employee's finger was unguarded at the time.

A maintenance employee was changing out the electrode tips of a welding machine. He was checking an electrode when the machine cycled. The electrode holder partially amputated his left index finger.

An employee was operating a punch press when the coils became jammed. The employee stood on the machine and reached for the coil. When the steel coil became unstuck, it sprung back and caught the employee's left middle finger, amputating it at the first knuckle.

An employee was setting up a die when the die started to fall. As the employee went to catch it, the die landed on and crushed the employee's right index finger between the die and the machine. Their right index finger was amputated slightly above the second knuckle.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
