An employee was cutting a pipe with a circular saw when the pipe pinched the blade and the blade shattered. A projectile from the blade struck the employee in the neck, lacerating him from the Adam's apple around to the ear.

Employees were putting in utilities during phase one of a subdivision construction/development. An employee was guiding a pipe that was being lowered/lifted into the ground by a backhoe. The pipe was attached to the backhoe with a chain. The employee's right hand was caught between the chain and the pipe. Their ring finger sustained a severe laceration/avulsion(s) requiring hospitalization and surgery.

An employee was cleaning a pugmill machine. The employee moved the machine belt with a 2-inch wrench when the belt caught his left sleeve and pulled his left arm into the belt, resulting in amputation of the arm.

An employee was walking backward to check on a sensor on an asphalt machine when he stepped onto a manhole cover that gave away. The employee slipped and fell onto the side of the manhole and the hot asphalt, resulting in a back injury and burns to his arms.

An employee slipped while putting a manhole ladder into the hole of a structure. The employee fell into the hole and landed about 14 feet below, hitting his head, chest, and leg on the ground. He suffered two cracked ribs, a foot injury, and head trauma and was hospitalized.

NATIONAL MILL MAINTENANCE, LLC

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

C. Miller Drilling Inc

A service crew was pulling a pump and motor from a well. After breaking a joint of pipe loose from another joint, an employee grabbed the chain tongs to prevent the pipe from spinning in the well, so the crew could continue taking it apart. The bolt that attaches the chain to the handle of the chain tongs broke, causing the chain to swing around and strike the employee's lower right leg. The chain severely punctured his leg and fractured his tibia.

An employee was using a handheld grinder to cut a bolt. The grinding wheel broke apart and a fragment struck the employee s right hand, causing a laceration to the palm near the little finger.

Blackburn's Fabrication, Inc.

An employee was operating a brake press when the top die broke, causing the bottom die to roll out and onto the employee's legs. Both of the employee's legs were crushed below the knee, and both legs were partially amputated. The employee's left leg was also fractured above the knee.

A pressurized aircraft nose landing gear strut assembly was being disassembled. The gland nut dislodged under pressure and made contact with an employee's left hand, breaking it.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.

An employee was assisting the crew with patching and restoring a roadway after the installation of a sewer line. He was helping place and level the blacktop/backfill material over the trench area to bring the road surface back to proper grade. The employee was walking between a forward-moving dump truck and a dirt pile when his right foot was run over by the truck s wheel, causing him to fall. The employee sustained injury to his foot.

An employee was rigging I-beams. An I-beam slid off the stack of I-beams and fractured the employee's leg.

An employee was operating a skid steer with a bucket attachment at a demolition site. While driving it to the rear of the property to park it, the employee came into contact with a live electrical transformer unit that was located outside of the facility. The employee sustained electrical burns on his hands extending up to his elbows and a fractured ankle.

Gerke Excavating, Inc.

An employee was installing a door on a semi-trailer. They were using a forklift to lift the door to the height of the trailer when the door became jammed between the forklift mast and the hinge on the trailer. When the employee went to dislodge the door from the mast, the door fell and crushed their foot. Their big toe was amputated. The employee was hospitalized and their big toe was surgically reattached. The employee's foot and other toes also required surgery.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.
