An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.

An employee was cleaning the mezzanine area when they crossed over the railing onto an I-beam and fell through a ceiling tile. The employee fell 15 feet to the floor and sustained a fractured spine and left heel.

On May 6, 2024, an employee was using a two-step stool to access a hopper and agitate the product. While stepping down, they missed the corner of the step and fell 20 inches to the ground. While falling, their right leg became entangled in the step stool resulting in a fractured tibia.

The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.
