Final narrative
On July 3, 2024, an employee was standing in front of a pallet getting material. A walk/ride pallet jack turned and pinned the employee's foot against the pallet. The employee sustained a fractured right ankle and was hospitalized.
An employee was operating a packaging machine when a plastic ribbon on the packaging film broke. The employee went to retrieve the broken ribbon when the machine cycled and contacted their right hand, resulting in amputation of the third and fourth fingers on the right hand. The machine was guarded at the time.
An employee was dispensing cleaning chemicals when the chemicals spilled on the floor and mixed together. While attempting to clean up the spill, the employee inhaled the fumes and experienced difficulty breathing.
A forklift turned into a warehouse side entry, and a rear tire caught an employee's foot and went over the employee's right leg. The employee suffered a closed fracture to the leg below the knee and was hospitalized.
