Two employees were demolishing a corkscrew conveyor in a grain silo. The chain fall one of them was holding fell and struck the side of the silo. This startled the other employee, who stepped off their step ladder, landed awkwardly on the ground, and broke their femur.

An employee was using a magnetic drill and caught their left thumb in the rotating drill bit resulting in a partial amputation.

An employee was walking through a parking garage. She tripped over a curb and fell forward onto her right knee, then her left knee, both hands, and her face. She suffered an injury to the left knee that required surgery, as well as scrapes on the chin and both palms and soreness in the left wrist.

An employee was approximately 5 feet up on a ladder while drilling a hole for cable installation. A wasp attacked him. As he was coming down the ladder, he landed flat on his feet on the ground, and his right leg was twisted. The employee sustained a fractured tibia.

An employee was using a scissor lift to demolish an air duct. The employee stepped onto the top railing to cut the air duct and then went to step down, lost his footing, and fell approximately 10-12 feet. The employee sustained three fractures on the left side of his pelvis and four back fractures.

An employee was doing a post-construction inspection after the telephone company worked on a pole. The employee put a 17-foot fiberglass stick against the pole, and was strapping a bungee cord around the pole so it wouldn t fall, when he contacted a faulty ground wire and sustained an electric shock.

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

FLATIRON CONSTRUCTION CORP.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

Takkion OPS Management

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
