Final narrative
On July 9, 2024, an employee was preparing to change out the transfer pump to the horizontal tank. When he adjusted the rear of the pump, it slid off its base and fell and crushed the employee's left index fingertip, resulting in amputation and loss of the nail.

An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.
An employee was unloading frameless mirrors from a trailer. When unstrapped, the mirrors fell over onto the employee, who suffered a skull fracture, a brain bleed and blood clot, a broken left ankle, and lacerations.

On May 6, 2025, a temporary employee was loading a sheet of fiberglass between two rollers on a roofing material machine. The top roller was not rotating. The roller fell and pinched his left ring fingertip, causing a partial amputation of the finger that required surgery.
An employee was about to dump sand from a hopper, raised on the forks of a forklift. As the hopper's weight shifted forward and it began to dump, the employee's left middle finger was pinched between the forklift fork and the metal frame of the hopper.
