An employee was cleaning small pieces of material off a fan when the fan cut his fingers, resulting in amputations to his thumb, index finger, and middle finger.

An employee was attempting to run paper bags through a printing press. He jogged the paper into place. His left hand was on the press cylinder when it rolled, amputating his left thumb below the nail bed and crushing his left index and middle fingers.

In the printing department, an employee's right hand, third and fourth fingers were amputated while operating a flexographic press.

An employee was moving a paper roll with an overhead crane. Her right pinky finger got caught and was amputated between the paper roll shaft and the bracket.

An employee was activating the sprinkler system at a sports complex following recent maintenance to repair leaky pumps. When the system was activated, a section of newly installed 3-inch schedule 80 PVC pipe struck the employee, who lost consciousness. The employee suffered a broken jaw and a concussion.

An employee was cutting metal with a torch. A piece of metal struck the employee's left foot, causing multiple fractures to metatarsal(s). The employee was hospitalized.

An employee had been conducting crane operations. The employee went to stop a piece of oil and gas equipment for tubular running from falling over to the ground. Their right hand was caught between a rolling ladder and the equipment, resulting in fractures to the index and middle fingers. They were hospitalized and the middle finger required surgery.

An employee was repositioning a large casting with a chain. The chain slipped and landed on his hand, resulting in hospitalization with a fracture to his upper arm and a crushing injury to his thumb.

An employee was installing a rear suspension airbag beneath a hydro-excavation truck when the airbag slipped off the truck's frame and struck him in the face. The employee sustained multiple facial fractures and an intraparenchymal hemorrhage and was hospitalized.

An employee was working to clean a glue roller with a scraper tool. The tool and the employee's left thumb were pinched between two rollers, resulting in a partial amputation.

An employee was shadowing coworkers while in training on the wide extrusion line. The employee went to clean the roller with a rag and cleaner when his right index finger got caught between a powered roller and a rubber roller. The employee sustained an amputation to a finger.

An employee was clearing a jam in a machine when it cycled. His hand was caught in the die, resulting in the amputation of two fingertips.

While operating a machine, an employee noticed that a piece of plastic material was stuck to a chrome roll. The employee was removing the plastic when their arm came into contact with the rollers. The employee suffered a contusion to the upper arm between the elbow and the shoulder.

On June 9, 2025, a machine operator was using a sorting machine to sort/clean plastic sheets. While she was freeing stuck sheets on the lift table, the table engaged and crushed her against the top railing, trapping her torso. The employee was hospitalized with contusions to her back.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
