Federal OSHA recorded a severe workplace injury at Clear Channel Outdoor, 565 Washington St, BRIGHTON, MASSACHUSETTS 02135 on — Fractures , affecting the Foot (feet) excluding toes.

An employee was standing on a rooftop using rope to lower a bundled advertising copy to the ground to a second employee. The rope got tangled in the employee's harness/lanyard or foot, causing the employee to fall approximately 15 feet to the sidewalk below. The employee landed on their feet and fractured both heels. The employee was hospitalized.

An employee was welding steel to a column and suffered an electric shock. The employee's gloves were wet at the time.

Buckeye Surface Maintenance

An employee was operating an asphalt grinding machine when a branch got in the way of the machine. He crawled on top of the machine to cut the branch but it snapped back and knocked him off the equipment. He fell approximately 10-12 feet to the ground below, resulting in fractures to both wrists and forearms, and a left knee fracture.

Rough & Ready of Central Florida Inc

A temporary employee was on a ladder while cutting conduit on a new building using a reciprocating saw. They sustained an electrical shock from a live electrical cable. The employee fell backward approximately 6 feet to the ground, resulting in hospitalization with a fractured heel that required surgery.

RIGGINS-MORELAND ENGINEERING INC.

An employee was working on the ninth position of a 12-foot ladder, installing a new light fixture in an existing building. The employee sustained an electrical shock, lost his balance on the ladder, and fell. He sustained a fracture of the lower back and bruises on his hand and foot. The fracture required surgery.

Industrial Fabrics, Inc.

At approximately 10:00 AM, employees were conducting asphalt loading operations. While securing a hose atop a truck, an employee encountered a malfunction involving a plugged hose. Pressure built up within the hose and suddenly released, causing the hose to fly off the truck. The hose contacted two employees and both employees fell approximately 7 feet. Employee 1 was on top of the truck and sustained three fractured ribs and a head contusion. Employee 1 was hospitalized. Employee 2 was climbing a ladder on the truck and sustained a fractured right leg. Employee 2 was not hospitalized.

Sentry Electric, Inc. of Central Florida

An employee was working from a ladder, installing light fixtures. The employee was shocked and fell from the ladder to the floor about 6 feet below, striking their head and back. The employee was hospitalized with a laceration to the head and broken vertebrae.

DISH Wireless L.L.C.

An employee began experiencing hand and arm cramping while climbing a tower. About an hour later, the employee became ill. The employee was hospitalized for heat-related illness.

An employee was working on a satellite installation. They were reaching to grab a piece of cable when they were bitten on the left hand by a copperhead snake.

An employee was climbing up a fixed wooden access ladder to the ceiling of a residential house. One of the ladder rungs failed, causing the employee to fall to the ground. He sustained a spiral fracture to his left leg.

An employee was replacing an old drop cord on a power pole. The employee was descending a ladder when they fell approximately 10 to 12 feet, resulting in a fractured pelvis.

On April 26, 2023, at 9:50 AM, an employee was installing a ladder rack on a company vehicle when the lowering arm of the ladder rack shifted, causing a bolt to slip and catch the wedding ring on his left ring finger. The employee sustained a laceration/avulsion to the left ring finger between the first and second knuckles that required hospitalization.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

Two employees were setting up a mandrel in the spindle of a machine. The mandrel was side-shifted while an employee's hand was on the mast of a powered industrial truck. The employee suffered crushing injuries that resulted in amputations to the middle and ring fingertips.

An employee had been loading a double deck press. The employee saw a mold had been sent in with the C-hook still attached on the belly bar. As he went to remove the hook from the mold, his right thumb got caught on the C-hook. The employee's thumb was partially amputated.

Warner Brothers, LLC

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

Roby's Propane Gas

An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.
